Directions: Answer YES or NO to each question:
- Do you find it difficult to remain calm when someone disagree with you?
- Do people describe you as two different people when calm vs. angry?
- Do your spouse and/or friends try not to argue with you?
- Do you yell and or raise your voice when upset?
- Would others describe you as defensive or sensitive to criticism?
- During an argument have you ever grabbed, shook, choked or hit anyone?
- Have you become so angry in traffic that you have tried to cut off, tailgated or rolled down your window to yell at others?
- Have you ever broken, hit or damage an object (wall, door, glass, chair, vase) during an argument?
- Have you ever surprised yourself by how angry you got and how you acted?
- Have you ever lost a relationship (partner or friend) because of your anger?
- Have you ever lost a job or been disciplined at work because of anger?
- Has anyone ever said that s/he is afraid of you?
- Have you ever been "blind" with rage, or could not remember what you did when angry?
- Have ever slapped or punched a loved one or child in anger?

- Have you shaken your fist or raised your hand toward someone?
- Do you find yourself having the same arguments over and over with your spouse, children or co-workers?
- Have you ever later regretted your actions after becoming angry?
Key:
- 0 YES Responses: Low probability that you have problem anger.
- 1-2 YES Responses: You may have a problem with handling your anger.
- More than 2 YES responses: A very strong indicator that you struggle with problem anger.
